Termination of Benefits
- Health – health insurance ends on the last day of employment.
- Dental – dental insurance ends on the last day of employment.
- Vision – vision insurance ends on the last day of employment.
- Basic Life – basic life insurance ends on the last day of employment.
- Supplemental Life – supplemental life insurance ends on the last day of employment.
- 401(k) – account will be moved to ‘Terminated’ status after the last paycheck contribution is received.
- Work Comp – coverage ends on the last day of employment.
- Incentive program – you are no longer eligible as of the last day of employment.
- Accruals – you stop accruing all paid time off benefits on the last day of employment.
