Reminder, you will need to continue notifying your supervisor of your need for leave by requesting your time off through the Duke payroll system.

Return to Work Requirements
Your employer requires you to report your return-to-work date a minimum of 48 hours in advance and provide specific information about any light duty restrictions that you may have upon return.

Compensation During Leave
PTO accruals (i.e. Vacation, Sick Leave, Paid Time off, Personal Holidays) will run concurrently with FML/Medical leave up to the last one week of Vacation, Personal Holiday, Paid Leave that you have available.

Benefit Payment Requirements
Duke will continue to deduct benefit premium payments from your payroll check while you are receiving them however any premiums that cannot be deducted from your check must be paid directly to Duke.

Medical leave and return to work information may be faxed to Duke 1-314-241-5528, emailed to HR@dukemfg.com or returned in person to your local HR team member.
