  • New employees are eligible for Duke Insurance benefits the 1st of the month following 30 days of employment.
  • Complete your 401k and Life Insurance beneficiary forms if you have not already and submit these documents to HR.
  • NEW HIRES have a one-time only offer of guaranteed coverage of additional life insurance up to $100,000 (self) and $40,000 (Spouse) WITHOUT COMPLETING A MEDICAL FORM (PHA) should you choose this option during new hire enrollment. This is the only time you will be able to request this coverage without completing a medical form. Should you choose an amount over this guaranteed coverage you are still required to complete the medical form (PHA). Every single time you choose to request a coverage increase from this date forward will require you to complete a medical form that will be submitted to the benefit provider for approval.
  • Every New hire at Duke is eligible for 401k on the 1st day of the month following 90 days of employment. You will be enrolled in an age-based plan at 6%. If you would like to make a change to this you can reach out to Fidelity at 1-800-Fidelity or 401k.com
