  • Health – you have the right to continue health coverage via COBRA. WageWorks will mail paperwork to you. If you elect coverage, it will become retroactive to the day you lost coverage through Duke and there will be no break/lapse in coverage.
  • Dental – you have the right to continue dental coverage via COBRA. WageWorks will mail paperwork to you. If you elect coverage, it will become retroactive to the day you lost coverage through Duke and there will be no break/lapse in coverage.
  • Vision – there is no option to continue vision coverage. Vision coverage ends on the last day of employment.
  • Basic Life – you have the right to convert your Duke-provided basic life insurance policy to a personal policy. Forms are attached.
  • Supplemental Life – you have the right to convert your supplemental life insurance policy to a personal policy. Form is attached.
  • 401(k) – After your last payroll-deducted deferral contribution is received by Fidelity, your account will be moved to ‘Terminated or Retired’ status. After your status changes to ‘Terminated or Retired’ you will be able to call Fidelity to find out all of the options you have for your 401(k) account. Fidelity’s customer service number is
    1-800-835-5097. Please note that True-Up calculations are performed on an annual basis. There may be an adjustment to your account after calculations are completed.
  • FSA – If you were contributing to an FSA account prior to your termination you have 90 days from termination date use any funds that you contributed however the claims must be dated prior to your termination. When/IF you elect coverage through cobra you can continue to contribute and use FSA funds. FSA Cobra coverage can only be elected through the end of the current plan year.
